The purpose of the Social Services Department of WestCare Health System is to identify patients discharge needs prior to admission and take steps to arrange for the assistance needed. Social Services staff is available to:

Discuss placement resources available according to patient care needs and financial resources for:

  • ECF (nursing home):
  • Patient receives skilled nursing care
  • Group homes (supervised living):
  • Patient must be able to ambulate independently
  • Patient must be able to perform ADL's
  • Facility provides no medical care (may have private home health)
  • Payment source may be SSI, county, private pay
  • Retirement homes:
  • Patient will have his/her own room/apartment
  • Facility may have communal dining
  • Patient must be independent-self-care (may have private home health)
  • Payment source is usually private pay
  • Family care homes
  • Custodial care provided
  • No medial care (may have private home health)
  • Payment source usually private pay; cost dependent on level of care needed

 

Arrange for patient's transfer by way of:

  • Air ambulance
  • Ground ambulance
  • Commercial airlines (with oxygen need 48-hour notice)
  • Transfer forms to be completed for all transfers

 

Make other referrals as appropriate for:

  • Home health (patient's choice)
  • DME (patient's choice)
  • Community resources

 

Make appropriate financial referrals (refer to social services for patient interview and assistance) for:

  • Medicaid
  • County
  • Medicare
  • Free Care
  • MediCal

 

Social Services can also make the appropriate referrals for:

  • Adoption
  • Adult and Child Protective Services Referrals
  • Medication Assistance Referrals
  • Counseling Services
