In choosing Hospice, patients and families make a decision to spend their last days together in a familiar and caring environment. With Hospice's support services, there's a sense of relief as trained professionals offer care and guidance. There is a promise of hope that is within the reach of patients and families who choose to take control of their situation with the help of Hospice. Hospice cares for both patient and family through the services of an interdisciplinary team that includes the attending physician, nurses, social service counselors, home health aides and volunteers. They are careful listeners, responsive caregivers and effective teachers. Decisions are made based on the individualized needs of patients, with the realization that something more can always be done to bring comfort and dignity to those dealing with a life limiting illness. Rather than a place, Hospice is a philosophy -- a program of care and support wherever patients need us. Hospice services extend to any location -- a home, the hospital, an adult program congregate living facility (ACLF) or a nursing home. No one need die alone as Hospice reaches out to any setting to provide full support. The Hospice support system continues after the death of a loved one with specially trained bereavement counselors assuming the support role. With the help of volunteers, they maintain a contact with family members for up to one year after a patient's death through visits, telephone calls and support groups.
